TERMS AND CONDITIONS YOU HAVE AGREED TO:
This document outlines the rights and responsibilities that you have with regard to the authority of AKTIVATE Yoga & Wellbeing of 9A, 64-86 Beresford Road, Lilydale 3140, to directly debit your nominated bank account or credit card for any instalments or fees due by you under the terms and conditions of the Contract and Service Agreement, the terms of which are stated below. Should you have queries regarding your Contract or this Direct Debit Request form you should in the first instance contact AKTIVATE Yoga & Wellbeing on phone 0400 536 040.

MEMBERSHIPS: Ongoing weekly membership will be active for a minimum of 3 months and continue indefinitely unless AKTIVATE Yoga & Wellbeing are notified via email accounts@aktivateyogawellbeing.com.au of a cancellation request. Cancellation of Membership prior to the minimum 3 month term will incur a cancellation fee of 50% of the reamaing term fee. Cancellation fee will need to be paid prior to pass cancellation.
5, 10 & 20 Class Passes and 6 Month passes are non-refundable.

MEMEBERSHIP PRICES: $29 per week - ($58 Fortnightly)

PAYMENTS: Weekly Memberships: AKTIVATE Yoga & Wellbeing will debit your nominated account for the amount in advance as agreed by you by accepting these Terms & Conditions, or any amendment to the amount of payment that is accepted by you.
5, 10 & 20 Class Passes and 6 Month Passes can be purchased via the website or in studio via eftpos or cash.

MEMBERSHIP SUSPENSION: Weekly Memberships payments can be suspended up to a total of 4 weeks in a 12 month period, with a minimum suspension of 1 week. This request must be lodged with at least 7 days notice of your suspension date, by emailing accounts@aktivateyogawellbeing.com.au. 
Additional holds within the 12 months can be purchased for $15.

Class Pass Holders can suspend their passes by request which must be lodged with at least 7 days notice of your suspension date, by emailing accounts@aktivateyogawellbeing.com.au. 
Additional weeks can be purchased for $15
5 Class Pass allows for 1 Week Suspension
10 Class Pass allows for 2 Week Suspension
20 Class Pass allows for 3 Week Suspension
6 Month Pass allows 4 Week Suspension

TIMETABLE:
Classes are subject to change or cancel without notice. 
Teachers are subject to change without notice. 
The timetable on public holidays and between Mid December – Mid January at AKTIVATE Yoga & Wellbeing may have a reduced timetable. It is the responsibility of the membership and pass holders to suspend their membership during this period.

MINORS:  
Children under the age of 18 are not permitted to practice public scheduled classes or workshops unless it is a specific/specialised Kids/Teen classes or advertised otherwise. 
Children and pets cannot be left unattended in the reception area for any reason at any time (including while practicing yoga). 

STUDIO ETIQUETTE
Please book into class online via this website or through PunchPass booking.
The studio is open 15 Minutes prior to the scheduled class time. Please respect your peers and keep talking/chatter out of the studio space and in the reception area only.
Please arrive at least 10 minutes prior to the listed start time for classes, workshops, courses and Reiki sessions.
Late entry to classes, workshops, and events is not permitted.
The front door to the studio will be locked at the listed time of classes, workshops, courses for safety and security - NO EXCEPTION

CANCELLATION: Membership holders may cancel this Contract by contacting AKTIVATE Yoga & Wellbeing in writing to accounts@aktivateyogawellbeing.com.au at least 7 days prior to your next direct debit.
AKTIVATE Yoga & Wellbeing reserve the right to cancel this arrangement if two or more drawings are returned unpaid by your nominated Financial Institution and an alternate payment method has not been arranged by emailing accounts@aktivateyogawellbeing.com.au
Should your payment be dishonoured, your bank may charge you a dishonour fee, and your membership will lapse until we are able to receive payment.

INCREASE IN FEES AND CHANGE OF TERMS: AKTIVATE Yoga & Wellbeing may at anytime upon sending written correspondence to your email address give you at least 14 days notice of any changes to fees or change of terms. If you have any questions on any proposed changes, please contact AKTIVATE Yoga & Wellbeing accounts@aktivateyogawellbeing.com.au

DISPUTES: all disputes regarding a direct debit payment should be referred to AKTIVATE Yoga & Wellbeing in person, by telephone 0400 536 040 or email accounts@aktivateyogawellbeing.com.au 

ENTITLEMENT: By signing this agreement you are agreeing to be bound by the rules and conditions of the Establishment. The Management at AKTIVATE Yoga & Wellbeing reserves the right to cancel the rights of any member not complying with the conditions of membership or rules of the Establishment at any time. Our services are provided to adults over the age of eighteen (18) years. By proceeding to purchase through our website, you acknowledge that you are over 18 years of age. 

PRIVACY: AKTIVATE Yoga & Wellbeing acknowledges and respects the privacy of individuals. The information that is being collected on this document is for the purposes of processing your membership and financial institution payments if applicable. You will receive communications from AKTIVATE Yoga & Wellbeing from time to time to update you on items relating to your membership. You have the right to access and alter personal information protecting yourself in accordance with the Commonwealth Privacy Act (Amended 2001).

LIABILITY: To the extent permitted by law, AKTIVATE Yoga & Wellbeing shall not be liable or responsible to you for any direct, indirect or consequential injury, loss or damage whatsoever and however arising. AKTIVATE Yoga & Wellbeing are not responsible for lost or stolen items (including personal yoga equipment) or damage to property or vehicles. Acknowledging this risk, you agree to use the Establishment at your own risk.

We at AKTIVATE Yoga & Wellbeing are not responsible for the safekeeping of your belongings and do not take accountability for lost or stolen items. 
Staff of AKTIVATE Yoga & Wellbeing have the right to refuse entry, or ask you to leave the premises at any time. 
By booking into a class, workshop, training, or other event you agree to these terms and conditions.
